What is Tesamorelin?
Tesamorelin is a GHRH analogue. It activates the GHRH receptor, which tells the pituitary to release growth hormone and raises downstream IGF-1 signalling.
Why people are interested
The potential benefits, unpacked.
Here are the goals and claims that bring readers to this guide. The evidence note shows which part is demonstrated, early research or still a question.
The headline interest
Randomised human trials show reduction in excess abdominal visceral fat in adults with HIV-associated lipodystrophy; liver-fat research is separate.
What the mechanism suggests
GHRH analogue stimulating pituitary GH and IGF-1; developed for a specific visceral-fat indication.
What still needs proving
The key question is whether the claimed benefit has been demonstrated in controlled human studies using the same compound, route and population.
What is it actually doing?
GHRH analogue stimulating pituitary GH and IGF-1; developed for a specific visceral-fat indication.
The important distinction is between changing a biomarker or pathway and producing a meaningful outcome that people can feel, measure or sustain.
Human evidence

The important studies, translated into normal English.
Each synopsis keeps the trial design, actual results and limitations together. Graphs below are redrawn from the cited numerical results so you can see the size of the effect without having to decode a journal figure.
Pooled Phase 3 HIV visceral-fat trials — 2010
Two Phase 3 trials · 806 patients · 26-week placebo-controlled phaseDaily 2 mg tesamorelin reduced visceral adipose tissue significantly; pooled treatment effect was −15.4% at week 26. Longer treatment sustained visceral-fat reductions, while stopping led to reaccumulation.
Tesamorelin has strong human evidence for a very specific problem: excess visceral abdominal fat in people with HIV.
What it doesn't prove: This does not establish it as a general weight-loss drug; total body weight and subcutaneous fat are different outcomes.

What doses and schedules are actually documented?
Human research or licensed product information is separated from clinic and community practice. These are source-attributed reference protocols, not personalised dosing recommendations.
Human research / licensed or product dosing
| Protocol / context | Amount & frequency | Duration | Titration | Evidence |
|---|---|---|---|---|
| Historical 1 mg-vial formulation | 2 mg SC once daily | Long-term, indication-based | Fixed | Medical/product history |
| Egrifta SV | 1.4 mg (0.35 mL reconstituted solution) SC once daily | Long-term | Fixed | Licensed formulation |
| Egrifta WR | 1.28 mg (0.16 mL) SC once daily | Long-term | Fixed | Current licensed formulation |
| Indication | Reduction of excess abdominal fat in HIV-infected adults with lipodystrophy | — | — | Licensed indication; not general obesity treatment |
Community / clinic / specialist protocols
| Pattern | Amount & frequency | Cycle | Progression | Status |
|---|---|---|---|---|
| Perfect B clinic protocol | 1 mg SC before bed, ≥2 h after food, 5 days on/2 off | 3 months then 2 months off | Fixed 1 mg; no ramp | Supervised clinic protocol |
| Prime Life standard | 1 mg SC nightly, 5 days/week | 12–16 weeks; 4–8 week washout | Fixed | Clinic/community |
| Prime Life maximum | 2 mg SC nightly, 5 days/week | 12–16 weeks; 4–8 week washout | Fixed | Clinic/community |
| Common research/community range | ~1–2 mg/day | 8–16 weeks | Usually fixed rather than titrated | Community |
Tesamorelin is a strong example of why 'units' should never be shown without mg and concentration: Perfect B's 25 units = 1 mg only because its example uses 10 mg in 2.5 mL. PP should always make milligrams the primary dose.

Routes and formulations matter. A topical study does not validate injection; an IV research dose does not establish a fixed SC dose; and evidence for one molecular form must not be silently transferred to another.
Community reports
What are people actually doing?
Community experience exists, but is anecdotal
Forums discuss schedules, effects and side effects, but posts are uncontrolled self-reports and product identity may be uncertain. Use them to understand what people report—not to estimate efficacy or establish a dose.

Established evidence is condition-specific and includes endocrine safety considerations.
Product identity, sterility and concentration are additional uncertainties for unlicensed research products. Evidence from an approved medicine or a clinical trial should not be transferred automatically to a differently sourced product.
Questions worth asking
What is the strongest evidence here?
Tesamorelin has strong human evidence for a very specific problem: excess visceral abdominal fat in people with HIV.
Does a biological mechanism prove the claimed benefit?
No. Receptor activity, cell effects and animal results can justify research, but controlled human outcomes are a separate evidential step.
Is the research protocol a recommended dose?
No. The site reports study protocols to explain the evidence. It does not turn them into personalised self-use instructions.
